Prospective Clients

Are you interested in joining the LaHaye and Doll ABA Services family? We currently have a waitlist in place for prospective clients. The process to initiate services is as follows:

STEP 1

Press the "Apply Now" button below to begin the process. You will be redirected to a new page.

STEP 2

Please upload medical documentation required by insurance for ABA services. You will be redirected to a new page.

STEP 3

Fill out all forms in the "Intake Packet & Required Documents" folder. You are responsible for sending the completed documents to the clinic via website upload, email, fax, drop off, or mail.

STEP 4

Once you have turned in forms, LaHaye and Doll ABA Services will be in contact with you to confirm or correct documentation and place on the waitlist!
